Parents/Guardians,
This Google Form should be completed in order to sign your child up for Band, Chorus, or Orchestra at the Nazareth Area Middle School (NAMS). Please complete all information required to ensure your child is scheduled for the correct groups for the school year.
Note: Students can be involved in more than one group. Band and Orchestra meet on the same day, but chorus will meet on the opposite day for Class.
